Project Lift is designed to remove barriers and identify solutions to enable families to reach self-sufficiency. This unique public/private partnership allows providers the flexibility, funding, and resources to meet a family’s needs quickly. Project Lift will track participating families for one and three years and use the information to implement new solutions to poverty.

To receive funding and resources from Project Lift, participating families must:

  • complete this form
  • collaborate with a case management provider
  • consent to monthly data collection for at least one year.
